C&S schließt Projekt zum Überleitungsmanagement zwischen Klinik und Pflegeeinrichtungen erfolgreich ab

Am 9.5.23 fand das BMBF-Abschlusstreffen "Digitale Plattformen: Interaktive Assistenzsysteme für den Menschen" in Berlin statt. Teil dieser Förderbekanntmachung war das Projekt Blog3 (Blockchain-basiertes Gesundheitsdatenmanagement für gesamtheitliche Gesundheitsprofile). Blog3 beschäftigt sich mit der Entwicklung einer digitalen Plattform, die die Integration von Gesundheitsdaten aus verschiedenen Systemen übernimmt und mit einem Blockchain-basierten Daten- und Rechteverwaltungssystem arbeitet. Der Fokus lag dabei auf dem Überleitungsmanagement zwischen Kliniken und nachsorgenden Einrichtungen. Die Aufgabe von C&S war insbesondere die Eingliederung in die Vernetzungs- und Integrationsplattform MCD.

Aktuell nutzt C&S die Expertise im Bereich Überleitungsprozesse in der sektorenübergreifenden Zusammenarbeit im vom Spitzenverband der gesetzlichen Krankenkassen geförderten Modellprogramm ITiV (Implementierung der Telematikinfrastruktur gem. §125 SGB XI in einem regionalen Versorgungsnetz der ambulanten und stationären Pflege in Berlin Nord-Ost).
